Several approach routes in EUS The EUS-guided rendezvous technique (EUS-RV) is a salvage method for failed selective biliary cannulation. The overall success rate of EUS-RV in 247 cases from seven published articles was 74 % Presented by Maurice E Arregui at the Video Face-Off Panel: Common Bile Duct Exploration - More than One Way to Skin a Cat held during the 2017 SAGES Annual 2018-08-22 · EUS-guided rendezvous technique (EUS-RV) has also been reported as an effective salvage technique for failed biliary cannulation during ERCP.[1–6] In EUS-RV, the biliary duct is punctured from the intestine under EUS guidance using a needle for fine needle aspiration (FNA), followed by guidewire placement into the duodenum through the needle, biliary duct and ampulla. Background and aims: Biliary cannulation is necessary in therapeutic ERCP for biliary disorders. EUS-guided rendezvous (EUS-RV) can salvage failed cannulation. Our aim was to determine the safety and efficacy of EUS-RV by using a standardized algorithm with regard to the endoscope position in a prospective study. EUS guided rendez-vous technique using 19g needle and ERCP guidewire to facilitate complex ERCP and biliary access in pancreatic adenocarcinoma with subseque in this video I describe how to identify the CBD using EUS scope then utilize EUS to advance a wire through the CBD across the ampulla. 3), first described in 2004, 40 the bile duct is accessed under EUS guidance with the creation of a temporary fistula, followed by guidewire advancement across the ampulla into the duodenum. Background and study aim Endoscopic ultrasound-guided rendezvous (EUS-RV) is increasingly reported as a treatment option after failed endoscopic retrograde cholangiopancreatography. We developed a novel "hitch-and-ride" catheter for biliary cannulation to reduce the risk of guidewire loss during EUS-RV. In 2004, EUS-guided biliary access for rendezvous ERCP was introduced as an additional option for patients who fail standard ERCP cannulation and drainage. 2, 3 Recent articles cite procedural success rates of 58 to 80%.

Design: Retrospective study.

A combined approach, also called a "rendezvous procedure" uses an x-ray guided puncture through the abdominal wall into the blocked bile duct, plus ERCP to

The route of access is anterograde, in contrast to the retrograde approach of ERCP. We have coined the term EUS-guided anterograde cholangiopancreatography

7,9 Among the various factors, a dilated PD seems to be essential for a successful EUS rendezvous procedure.

The first EUS‐RV report was by Mallery et al. 8 in 2004; EUS‐guided rendezvous drainage of the obstructed biliary and pancreatic ducts was performed in patients with ERCP failure as well as those with pancreatic drainage. Many reports followed, making the procedure widely recognized. EUS (UCT-180; Olympus Ltd, Tokyo, Japan) revealed a dilated CBD with multiple calculi without intrahepatic biliary radicle dilation . Attempts at biliary access during ERCP were unsuccessful because of nonvisualized intradiverticular papilla and duodenal deformity . Hence, EUS-guided rendezvous (EUS-RV) was considered. EUS rendezvous or direct intervention involves: (1) using endoscopic-ultrasound technology to access the bile duct with a small needle and manipulate a wire across the biliary orifice and into the duodenum to be then retrieved endoscopically for ERCP (rendezvous ERCP), or (2) using endoscopic-ultrasound technology to directly puncture and perform intended biliary therapy Background and study aims  The aim of this study was to assess the efficacy and safety of endoscopic ultrasound-guided rendezvous (EUS-RV) for benign biliary or pancreatic disorders with a 22-gauge needle and a 0.018-inch guidewire.

TABLE 1Summary of studies evaluating EGBD after failed ERCP. ∗. ∗ Only studies EGBD, EUS-guided biliary drainage; Ren, rendezvous; TL, transluminal.

OBSERVATIONS: EUS-guided transhepatic cholangiography was performed in 6 patients, with successful rendezvous ERCP and stent placement in 4, and transduodenal stent placement in another patient. Stent placement was unsuccessful in one patient, because of the inability to advance a guidewire into the common hepatic duct. The endoscopic ultrasound (EUS)-guided rendezvous technique was reported to be a useful salvage method for patients with failed cannulation.
